Notice of Privacy Practices

This notice describes how protected medical information you may be used and disclosed and how you can gain access to this information. The privacy of your health information is important to us. Bakare & Associates is required by law to maintain the security and confidentiality of your medical information as well as other identifiable information such as your name, address, and telephone number.

We are required to extend certain protections to your “Protected Health Information” (PHI), which includes any identifiable information about your past, present, or future health care services or payment for your health care. We are required by law to provide you with this notice regarding our legal duties and our privacy practices. Bakare and Associates are required to abide by the terms of the Health Insurance Portability and Privacy Act (HIPPA).

We reserve the right to change our Privacy Practices and the terms of this notice at any time. For example, if privacy laws change, we will change our practices to comply with the law. Bakare and Associates will be happy to provide you at your request, a copy of our Notice of Privacy Practices at the time of your appointment.


Most health insurance plans are intended to cover some but not all of your health care costs. Your insurance will probably have a co-payment, co-insurance, deductible or other expenses which must be paid by the patient. All patient expenses are expected at the time services are rendered.

If there is a problem or in cases of non insured patient, our Billing Secretary will be happy to discuss the situation and work out a payment arrangement if necessary. You will receive an office charge slip at the end of each visit. Where a payment is made this is your receipt. Annual itemized statements are not provided. If you have any questions about your account, please call our Billing Secretary. We participate in most health care plans in Central Pennsylvania area.
